CMS Facility Request * - Coaches need to fill out the attached form and fax it to Deb Weir. Deb will then submit the form to the school district for approval and scheduling.

Coaching Supervision Policy - This provides a description of WLA's expectations regarding the supervision of players by each team's designated coaching staff.

Emergency Info and Consent Form * - Coaches should distribute this form to parents at the beginning of the season keep the signed documents with them at all WLA activities.
